The Challenge

The Children’s Hospital of Orange County, CHOC, is dedicated to focusing on Population Health, and has always been a leader in progressive care.

Our Process

Our bidirectional API platform has allowed physicians at CHOC to collect data in real-time from its many independent clinics that operate inside its Clinically Integrated Network. This data is collected, merged, then distributed into CHOC’s Population Health Platform to assist in facilitating faster, more accurate care for patients.

Results

CHOC is the only hospital in Orange County to be awarded government funding of close to 18 million dollars to focus on Population Health, and with our help, CHOC’s statistics about care for asthma patients have greatly improved, helping the hospital to continue its mission for quality health care. This collaboration was an overwhelming success, with the proof presenting itself in the data.
